A direct hire opportunity is available for a Clinical/Infusion Pharmacist specializing in home infusion therapies. This role involves preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, serving as a pivotal member of a multidisciplinary team to ensure optimal patient care. The position is based in Hilliard, OH, with a Monday through Friday schedule, 8:00 AM to 4:30 PM, including participation in an on-call rotation.

Key Responsibilities:

  • Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions in accordance with strict sterile techniques and regulatory standards.
  • Clinical Monitoring: Assess laboratory results, perform pharmacokinetic dosing calculations (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and provide patient counseling on specialty medications and treatment plans.
  • Patient Education: Instruct patients and caregivers on the use of infusion pumps, sterile procedures, and catheter maintenance.
  • Care Coordination: Work collaboratively with physicians, home health nurses, and internal teams to ensure effective therapy management and reduce hospital readmissions.
  • Regulatory Compliance: Adhere to relevant regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
  • Inventory Management & Shipping: Oversee pharmacy inventory, including temperature-sensitive products, and coordinate the packaging and delivery of medications for home administration.

Qualifications:

  • Minimum of 2 years’ experience in infusion specialty pharmacy.
  • Familiarity with IVIg therapy (preferred).
  • Knowledge of specialty therapy payer requirements, reimbursement procedures, and contract obligations.
  • Experience with CareTend system.
  • Current and active Ohio pharmacist license in good standing.
