• Assess the functional capabilities of residents.
• Design and execute personalized treatment plans.
• Assist residents in achieving greater independence in daily activities and mobility.
• Work collaboratively with nursing, rehabilitation, and care teams to provide safe, individualized interventions.
• Offer training to caregivers and document residents' progress.
• Participate in discharge planning and overall care strategies.
