Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A direct hire opportunity is available for an experienced Clinical/Infusion Pharmacist in Syracuse, NY, specializing in home infusion therapy. This role requires a dedicated clinical pharmacist to prepare, dispense, and monitor complex intravenous (IV) and subcutaneous medications, playing a critical role within a multidisciplinary healthcare team.
Schedule/Hours:
- Monday to Friday, 8:00 AM to 5:00 PM
- Participate in an on-call rotation (typically fewer than 5 after-hours calls per week)
Key Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in a sterile environment, ensuring compliance with state and federal standards.
- Monitor clinical parameters including lab values and pharmacokinetic dosing, particularly for therapies such as vancomycin and Total Parenteral Nutrition (TPN).
- Provide detailed patient and caregiver education on infusion pump operation, sterile techniques, and catheter maintenance.
- Collaborate with healthcare providers and home health nurses to coordinate care, optimize therapy, and reduce hospital readmissions.
- Ensure ongoing regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant bodies.
- Manage pharmacy inventory, including temperature-sensitive products, and prepare medications and supplies for prompt patient delivery.
Qualifications and Experience:
- Minimum of 3 years in an infusion specialty pharmacy setting.
- Strong preference for candidates with IVIg and hemophilia experience.
- In-depth understanding of specialty therapy payer requirements, reimbursement procedures, and contract terms.
- Proficiency with CareTend system.
- Current and valid NY pharmacist license in good standing.
