Hospice Licensed Practical/Vocational Nurse (LPN/LVN)

We’re seeking caring and dependable Hospice LVNs to support patients and families during end‐of‐life care. In this role, you’ll work closely with RNs and the interdisciplinary hospice team to deliver comfort‐focused, patient‐centered care that honors individual needs and wishes.
Schedule
- Per Diem
- Flexible shifts ranging from 4 to 12 hours
- Day, evening, or night shifts based on assignment needs
What You’ll Do
- Provide direct hospice nursing care under the supervision of a Registered Nurse
- Assist with symptom management, comfort measures, and palliative treatments
- Administer medications and treatments per physician and RN orders
- Monitor patient condition and report changes promptly to the RN and care team
- Provide emotional support and reassurance to patients and families
- Reinforce patient and family education related to care plans and symptom management
- Collaborate with physicians, social workers, chaplains, and interdisciplinary team members
- Support care coordination across home hospice or inpatient hospice settings as assigned
- Document care activities, observations, and patient responses accurately in the medical record
What We’re Looking For
- Active LVN license in the state of practice
- Minimum of 1 year of recent hospice, palliative care, or acute care LVN experience required
- BLS certification required
- Compassionate, patient‐advocate approach to care
- Strong communication and clinical observation skills
- Ability to support patients and families through emotionally complex situations
- Comfortable working independently while collaborating with interdisciplinary teams
- Travel may be required for home hospice assignments
- Inpatient hospice roles are facility‐based and do not require travel
