We are looking for a dedicated and empathetic Physical Therapist to become a part of our Healthcare & Medical Services team. This position is based on-site, where you will deliver evidence-based rehabilitation services to our senior residents across various living arrangements, including independent living, assisted living, and memory care.

Key Responsibilities

• Conduct thorough assessments of residents' physical conditions.
• Design and implement personalized treatment plans tailored to individual needs.
• Develop and oversee fall-prevention and mobility enhancement programs.
• Work closely with nursing and care staff to ensure comprehensive resident care.
• Provide education and support to residents and their families.
• Maintain accurate documentation of care in electronic health record (EHR) systems.
