Home Infusion Clinical Pharmacist | Dublin, Ohio

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A direct hire opportunity is available for a Clinical/Infusion Pharmacist in the Dublin, OH area, supporting home infusion therapies. This role is ideal for pharmacists with a strong background in infusion specialty pharmacy who thrive in multidisciplinary team settings and patient-centered care.

Schedule & Location:

• Monday through Friday, 8:00 a.m. to 4:30 p.m.
• Participation in on-call rotation
• Dublin, OH

Key Responsibilities:

• Prepare and dispense specialty pharmaceuticals and intravenous (IV) solutions in a sterile, ventilated environment, adhering strictly to state and federal regulations.
• Perform clinical monitoring, including assessment of lab values, pharmacokinetic dosing calculations (e.g., vancomycin), and adjustment of Total Parenteral Nutrition (TPN) therapies.
• Provide detailed patient and caregiver education on infusion pump usage, sterile technique, and catheter care.
• Collaborate with prescribing physicians, home health nurses, and internal teams such as intake, reimbursement, nursing, and pharmacy operations to ensure seamless therapy management and prevent hospital readmissions.
• Maintain compliance with all regulatory standards from the State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, among others.
• Oversee inventory management, including refrigerated products, and prepare medications for safe and timely delivery to patients’ homes.

Qualifications:

• Minimum of 2 years’ experience in infusion specialty pharmacy
• IVIg experience is a plus
• Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
• Proficient with CareTend system
• Current Ohio pharmacist license in good standing
