Home Infusion Clinical Pharmacist in Syracuse, NY
Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A direct hire opportunity is available for a Clinical/Infusion Pharmacist based in Syracuse, NY, starting ASAP. This role involves providing expert pharmaceutical care in the home infusion setting, supporting patients requiring complex intravenous (IV) and subcutaneous therapies.
Work Schedule:
- Monday to Friday, 8:00 AM – 5:00 PM
- Participation in an on-call rotation (typically fewer than 5 after-hours calls per week)
Key Responsibilities:
- Prepare specialty pharmaceuticals and sterile IV solutions in a compliant, ventilated environment according to state and federal regulations.
- Monitor patients’ lab values and calculate pharmacokinetic dosing (e.g., vancomycin) and manage Total Parenteral Nutrition (TPN) dosing adjustments.
- Provide comprehensive counseling to patients and caregivers on specialty medications and treatment regimens.
- Educate patients and caregivers on the use of infusion pumps, sterile techniques, and catheter maintenance for home care.
- Collaborate with physicians, home health nurses, and multidisciplinary teams to coordinate therapy and reduce hospital readmissions.
- Maintain regulatory compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
- Manage pharmacy inventory, including refrigerated products, and ensure safe packaging and timely delivery to patients.
Desired Qualifications and Experience:
- Minimum of 3 years’ experience in infusion specialty pharmacy.
- Experience with IVIg and hemophilia treatments is highly preferred.
- Strong knowledge of specialty therapy payer requirements, reimbursement, and contract obligations.
- Proficiency with CareTend system.
- Current and in good standing NY pharmacist license.
