Long Term Care Mail Order Staff Pharmacist - Saint Paul, MN

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This opportunity is for a Staff Pharmacist position working within a Long Term Care (LTC) Mail Order pharmacy environment on a 13-week contract. The role involves variable shifts Monday through Friday (9:30 AM–6:00 PM / 10:00 AM–6:30 PM / 11:00 AM–8:30 PM) and weekend shifts (Saturday and Sunday 10:00 AM–6:30 PM), with schedules varying weekly.

Responsibilities:

• Accept and dispense prescription medications accurately and responsibly.
• Monitor patient medication compliance and identify any patterns of over-usage, ensuring legal adherence for medication supplies.
• Verify that prescribed medicines are appropriate for patients’ conditions.
• Maintain confidential and accurate records of patient information and pharmacy files.
• Provide counsel to patients regarding medication use, potential side effects, and answer any medication-related questions.
• Offer consultations on prescription and non-prescription drugs, covering topics like storage, dosages, and drug interactions.
• Deliver patient services such as smoking cessation advice, blood pressure measurement, and cholesterol management.
• Educate patients on proper use and maintenance of medical devices, including blood pressure monitors and glucose meters.
• Compound medications for both external and internal use, preparing sterile solutions following established protocols.
• Supervise pharmacy personnel, including pharmacy technicians and clerks.
• Manage inventory of pharmaceutical and medical supplies.
• Identify drug-related issues in patient therapy and communicate effectively with physicians and other healthcare providers.
• Collaborate closely with healthcare teams including physicians, nurses, pharmacy interns, and others.

Qualifications:

• Doctor of Pharmacy degree required.
• Minnesota Pharmacy license required.
• Experience in Long Term Care pharmacy preferred.
• IV compounding experience is a plus but not mandatory.
