Home Infusion Clinical Pharmacist - Baldwinsville, New York
Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A home infusion clinical pharmacist role is available for a direct hire position in the Baldwinsville, NY area with a focus on clinical pharmacy and specialized home infusion services. This role offers a Monday through Friday, 8 a.m. to 5 p.m. schedule with participation in an on-call rotation averaging fewer than five after-hours calls per week.
Key Responsibilities:
- Sterile Compounding: Prepare specialty pharmaceuticals and intravenous (IV) solutions in a sterile, ventilated environment adhering to all applicable state and federal regulations.
- Clinical Monitoring: Evaluate lab results, calculate pharmacokinetic dosing such as vancomycin levels, and monitor Total Parenteral Nutrition (TPN) to optimize treatments. Provide patient counseling and education regarding specialty medication regimens.
- Patient Onboarding & Education: Teach patients and caregivers proper use of infusion pumps, sterile techniques, and catheter maintenance for home care.
- Care Coordination: Work collaboratively with prescribing physicians, home health nurses, and interdisciplinary teams including intake, referral services, reimbursement, nursing, clinical, and pharmacy operations to ensure seamless treatment management and reduce hospital readmissions.
- Regulatory Compliance: Ensure all pharmacy processes comply with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards.
- Inventory & Shipping: Manage pharmacy inventory, including refrigerated products, and ensure accurate packing and timely delivery of medications and supplies to patients’ homes.
Qualifications:
- Minimum of 3 years’ experience in infusion specialty pharmacy
- Strong preference for candidates with IVIg and hemophilia experience
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
- Current and valid New York pharmacist license in good standing
- Experience with CareTend pharmacy system preferred
