\n The Role\n

\n
• Manage a caseload of six bespoke complex care packages.
\n
• Complete comprehensive clinical assessments and develop person-centred care plans.
\n
• Provide clinical leadership and support to care teams, ensuring the highest standards of care.
\n
• Work collaboratively with families, CHC teams, commissioners and community professionals.
\n
• Monitor clinical governance, medication management and care quality across your caseload.
\n

About You\n

\n
• Registered Nurse (NMC).
\n
• Experience supporting children within a community or complex care setting.
\n
• Experience caring for clients with tracheostomies and mechanical ventilation.
\n
• Full UK driving licence and access to your own vehicle.
\n
