Our facilities function as Skilled Nursing Facilities (SNFs), offering:

  • Long-term care for residents needing continuous skilled support and compassionate daily assistance.
  • Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
  • Collaborative care through a team-oriented approach involving nursing, therapy, and medical staff working in unison.
  • A diverse patient demographic that includes individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs.

The patient mix may vary slightly by facility. As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life within a supportive, patient-focused environment.

Position Overview

The Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Education, Experience, and Licensure Requirements
  • Graduation from an accredited speech therapy program (minimum MS or MA SLP).
  • ASHA certification (CCCs) OR eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCCs) and/or meet ASHA credentialing criteria. Must maintain ASHA credentials once obtained.
  • Current registration/licensure in the applicable state, with an active license in good standing throughout employment.
  • Preferred one (1) year of experience; new graduates are encouraged to apply.
  • CPR certification required upon hire or to be obtained during orientation, with the need to keep it current throughout employment.
Specific Job Responsibilities
  • Engage in community health initiatives/projects as appropriate.
  • Proficient in Microsoft Word, Excel, and email communication.
  • Collaborate with patients, families, and support departments to effectively address patient needs.
  • Exhibit proper body mechanics at all times.
  • Demonstrate the ability to make independent decisions when necessary.
  • Familiar with practices, procedures, and regulations governing operations in a post-acute care setting.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Perform competently in all areas, including patient rights, safety, and sanitation.
  • Maintain professional relationships with colleagues, vendors, and others.
  • Ensure confidentiality of all proprietary and sensitive information.
  • Adhere to organizational policies, including those related to harassment and compliance.
  • Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing mandatory compliance training.
Essential Functions
  • Establish, evaluate, and adjust realistic, measurable, and functional goals.
  • (SLP CCC only) Oversee and assess care provided by SLP CFYs.
  • (SLP CFY only) Follow speech therapy treatment plans under the guidance of the supervising SLP CCC, in accordance with Federal and State regulations.
  • Document appropriately and in a timely manner.
  • Utilize therapy software accurately and effectively.
  • Provide excellent customer service and maintain a positive attitude towards patients.
  • Assist in the safe evacuation of patients when necessary.
  • Demonstrate reliable and consistent attendance.
  • Employ reasoning skills and sound judgment in all situations.
  • Communicate effectively and collaborate productively within an interdisciplinary team.
  • Engage in various physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
  • Read, write, speak, and comprehend the English language.
