St. Joseph St., Green Bay, Wisconsin 54301. This community has a unique setup and known as The Village of Eden Vista and The Court of Eden Vista. It is essentially two facilities on the same block. The Village is an RCAC with a capacity of 50 units. The Court is a 69-bed memory care facility that specializes in residents who have dementia.\n

\nEden Vista Green Bay is seeking a compassionate and dedicated Med Passers to join our team in Green Bay, WI. The ideal candidate will be passionate about providing exceptional care to our residents, ensuring their safety, comfort, and well-being in a warm and supportive environment. As a Caregiver, you will play a vital role in enhancing the quality of life for our residents through personal care, companionship, and assistance with daily activities.\n

\nJob Description\n
\n
\nProvide kind, caring, and consistent companionship and support.\n
\nProvide assistance, encouragement, and restorative interventions, as needed, for Memory Care residents to be as independent as possible in their activities of daily living including but not limited to bathing, dressing, grooming, toileting, feeding, and mobility.\n
\nParticipate in and lead one on one and group activities.\n
\nServe residents during mealtime, assist with memory care laundry, and basic housekeeping within the memory care neighborhood.\n
\nAdhere to and is a champion for the Embrace Memory Care philosophy.\n
\nDocument daily according to memory care policies and procedures and state regulations, including but not limited to, the communication of any resident concerns, changes in condition, or related topics.\n
\nEnsure that SLC policy and procedure are followed to maintain a safe work environment through the prevention of accidents, the preservation of equipment, and training of staff on safe working practices.\n
\nEnsure that resident/family/employee concerns receive a timely response and follow up, thus striving to provide a high level of customer satisfaction.\n
\nPerform all other duties as assigned.\n

\nExperienced in long-term care or Assistant Living\n
\nAble to communicate well in both verbal and written forms\n
