Pay Rate:£13.93 per hour plus 28p mileage

\n

Shifts available:07:00 - 22:00, four days per week + on-call for additional payment of up to £250.00 per week

\n

Shift Pattern:Week 1: Monday, Tuesday, Thursday, FridayWeek 2:Monday, Friday, Saturday, Sunday

\n

Qualifications\n
\n

You would need experience in carrying out initial risk and needs assessments in respect of individual service users, either in the service users home or, where required, in other settings such as hospitals or residential/nursing homes; Work in partnership with service users, their family, carers, advocates, healthcare professionals and others to ensure the seamless delivery of holistic packages of individual care support.

\n
