Job Description

As a Social Worker, you can be the reason why lives change for the better, helping some of the most vulnerable people gain a brighter future.With previous experience from the NHS or private care, you can join the multidisciplinary team at The Woodmill, Cullompton, which has a strong emphasis on teamwork and where you will provide care for patients as they step down through their recovery.As a Social Worker you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitating group activities, and contributing to ICR meetings. You will support service users to make viable plans for their future.You will report to the Safeguarding Lead and will manage internal investigations and liaise with home authorities for service users during their admission and discharge process. It is what delivering great healthcare should feel like.As a Social Worker you will be:Lone workingWork closely with other MDT membersWrite and complete statutory reports (CPA, MH) within specified timelinesWork with families, creating a therapeutic and professional relationship beneficial to the patientReferring to LADO and Local Authority in line with safeguarding and assessmentsEducating hospital staff about relevant safeguarding mattersCompleting a full annual social care reassessment within the CPA formatFacilitating access to welfare rights, housing, advocacy services and associated support servicesParticipating in 1:1 supervisionTo be successful in this role, you will need:A Social Work degreeExperience within a similar environmentExperience working with vulnerable adultsWhere you will be working:Location: Exeter Road, Cullompton, Devon, EX15 1EAThe Woodmill is an 18-bed unit that creates a pathway for patients to step down through their recovery.
