Deliver expert clinical care as a Sterile Compounding Pharmacist focusing on home infusion therapy in Frisco, TX. This contract opportunity spans 13 weeks and requires immediate availability to ensure seamless patient support and medication management within a specialized pharmacy environment.

Key Responsibilities:

• Evaluate patient eligibility for home infusion therapy and make clinical decisions based on assessments and laboratory data.
• Develop and adjust personalized medication therapy plans through detailed clinical monitoring and pharmacokinetic analysis.
• Prepare, verify, and dispense sterile compounded medications adhering to USP standards and all regulatory requirements.
• Educate patients, caregivers, and healthcare professionals on medication administration, infusion processes, and proper drug use.
• Collaborate with healthcare teams to maintain continuity of care and monitor treatment outcomes.
• Manage infusion pumps and related equipment, maintaining strict quality assurance and safety protocols.
• Provide comprehensive drug information, maintain meticulous documentation, and support medication safety and regulatory compliance efforts.

Qualifications and Desired Experience:

• Doctor of Pharmacy (PharmD) degree or graduate from an accredited School of Pharmacy.
• Active, unrestricted Pharmacist license in Texas.
• Demonstrated sterile IV compounding experience of at least 1 year, OR experience in non-sterile compounding, immunizations, MTM, completion of an ASHP-accredited residency, APPE completion, or equivalent pharmacy experience.
• Strong clinical knowledge of home infusion pharmacy practices.
• Proficiency in interpreting laboratory values and applying pharmacokinetic principles.
• Exceptional communication, patient counseling, and customer service abilities.
