Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This opportunity is ideal for a licensed pharmacist with expertise in sterile compounding and clinical pharmacy practices focused on home infusion therapy. The role involves working a 13-week contract with a consistent schedule from 12 PM to 8 PM in Cibolo, TX, delivering patient-centered pharmacy care in a home infusion setting.

Key Responsibilities:

  • Assess patient eligibility and suitability for home infusion therapy.
  • Develop, monitor, and adjust personalized medication therapy based on clinical assessments, lab values, and pharmacokinetics.
  • Prepare, verify, and dispense sterile medications adhering to USP standards, regulations, and pharmacy policies.
  • Educate patients, caregivers, and healthcare providers on medication use and infusion procedures.
  • Collaborate with healthcare providers to ensure continuity of care and monitor patient progress.
  • Manage infusion pumps and related equipment, maintaining safety and quality assurance.
  • Provide drug information support, maintain documentation, and uphold medication safety and regulatory compliance.

Qualifications and Experience:

  • Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
  • Active, unrestricted Texas Pharmacist license.
  • At least one of the following:
    • One year minimum of sterile IV compounding experience.
    • Experience in non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
    • Completion of an ASHP-accredited pharmacy residency.
    • Completion of an Advanced Pharmacy Practice Experience (APPE).
    • Equivalent relevant pharmacy experience.

Skills:

  • Strong clinical knowledge of home infusion pharmacy.
  • Proficiency interpreting lab results and pharmacokinetic data.
  • Excellent communication, patient counseling, and customer service skills.
