Our organization, a home healthcare provider located in Connecticut, is looking for a dedicated Registered Nurse Post Acute Transition Specialist. This role focuses on ensuring smooth and safe transitions for patients moving from hospitals or facilities back to their homes.

Key Responsibilities
  • Conduct thorough assessments to evaluate patient needs.
  • Develop personalized care plans tailored to individual patient requirements.
  • Provide education and support to patients and their families regarding care processes.
  • Work collaboratively with hospitals, physicians, and our interdisciplinary team to minimize readmission rates.
  • Facilitate recovery by ensuring continuity of care during the transition period.
Qualifications and Requirements
  • Current Registered Nurse license in Connecticut.
  • Experience in home healthcare or a related field is preferred.
