Private Duty Nurse (LPN / RN)

Location: Grand Rapids, MI
Job Type: Full-Time / Part-Time (Onsite)
Practice Setting: Private Duty Home Care

We are seeking compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide one-on-one skilled nursing care in patients' homes. This role focuses on delivering high-quality, patient-centered care while promoting patient safety, comfort, and independence.

Candidates with home health, private duty, or long-term care experience are preferred. Ventilator and tracheostomy experience is a plus, but training is available for motivated candidates.

• Provide skilled nursing care in a home setting

• Perform patient assessments and develop care plans

• Administer medications, IV therapy, and treatments

• Provide wound care, tube feeding, catheter, and tracheostomy care

• Monitor patient condition and vital signs

• Educate patients and caregivers on treatment plans

• Maintain accurate clinical documentation and HIPAA compliance

• Collaborate with physicians and the healthcare team

• Active Michigan LPN or RN license required

• Home health, private duty, or long-term care experience preferred

• Ventilator, tracheostomy, wound care, and tube feeding experience preferred

• Strong clinical assessment and communication skills

• Compassionate, reliable, and patient-focused

• LPN: Based on experience

• RN: Based on experience
