JOB SUMMARY:
\nThe home health caregiver is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist. This position includes providing personal care services, observing and reporting patient conditions, and documenting care provided.

QUALIFICATIONS:

• \n\t

  Current California Certified Home Health Aide (CHHA) or Certified Nurse Assistant (CNA) certification

  \n\t
\n\t
• \n\t

  At least 18 years of age

  \n\t
\n\t
• \n\t

  Ability to read, follow written instructions, and document care

  \n\t
\n\t
• \n\t

  Self-directed with minimal supervision

  \n\t
\n\t
• \n\t

  Compassionate and empathetic toward the ill, frail, or impaired

  \n\t
\n\t
• \n\t

  Current CPR certification

  \n\t
\n\t
• \n\t

  Valid driver’s license and insured vehicle (per agency requirements)

  \n\t
\n\t
• \n\t

  Professionalism, patience, and good personal hygiene

  \n\t
\n

RESPONSIBILITIES:

  Adhere to agency policies and procedures

  Assist with personal care and hygiene

  Assist with transfers and ambulation
