IV Sterile Compounding Clinical Pharmacist - San Antonio, TX
Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
This opportunity is ideal for a clinical pharmacist specializing in sterile compounding and home infusion therapy. The contract position is based in San Antonio, TX, with a 13-week duration and a consistent schedule from 12 PM to 8 PM.
Key Responsibilities:
- Evaluate patients for eligibility and appropriateness of home infusion therapy.
- Design, monitor, and adjust personalized medication therapy plans using clinical assessments, lab values, and pharmacokinetics.
- Prepare, verify, and dispense sterile medications adhering to USP standards, regulations, and internal policies.
- Educate patients, caregivers, and healthcare professionals about medication regimens, infusion techniques, and safe administration.
- Collaborate closely with healthcare providers to ensure seamless continuity of care and monitor patient progress and outcomes.
- Manage and maintain infusion pumps and related equipment, ensuring quality assurance and safety standards.
- Provide accurate drug information, maintain thorough documentation, and support initiatives focused on regulatory compliance and medication safety.
Qualifications & Desired Experience:
- Doctor of Pharmacy (PharmD) degree or graduate of an accredited School of Pharmacy.
- Active, unrestricted Texas Pharmacist license.
- Minimum of one of the following:• At least 1 year of sterile IV compounding experience• Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM)• Completion of an ASHP-accredited Pharmacy Residency• Completion of an Advanced Pharmacy Practice Experience (APPE)• Equivalent pharmacy experience
- Strong clinical knowledge of home infusion pharmacy practices.
- Proficient in interpreting laboratory values and applying pharmacokinetic principles.
- Excellent communication skills, patient counseling expertise, and commitment to superior customer service.

- 13-week contract
- Shift: 12:00 PM – 8:00 PM
