Office Hours:
- Monday: 7:30 AM - 6:00 PM
- Tuesday - Friday: 7:30 AM - 5:00 PM
- Team Composition: 2 providers, 3 Advanced Practice Providers (APPs), 2 Licensed Master Social Workers (LMSW), 1 Pharmacist (Pharm D), 1 Care Manager, and 21 support staff.
- Teaching Opportunities: Yes, including Residents, Medical Students, and Fellows.
- On-Call Rotation: Physicians participate in an on-call rotation throughout the year.

Key Responsibilities- Provide high-quality, comprehensive care in a patient-centered environment.
- Utilize advanced digital tools such as Epic and MyChart for efficient patient management.
- Collaborate with skilled physicians, APPs, and dedicated support staff.
- Engage in quality improvement initiatives and support community health efforts.
- Active Michigan medical license (or eligibility to obtain one).
- Experience with documenting in Epic EMR.
- Open to new graduates.
