Contract: 08/10/2026 to 11/07/2026
Pay: $40/hr (Paid Weekly)
Travel:
100% travel required throughout the workday. This position involves traveling to multiple locations and providing services in patients' homes.
Position Overview
We are seeking a compassionate and organized Medical Social Worker to join our healthcare team in La Grande, Oregon. In this role, you will provide psychosocial assessments, care coordination, patient advocacy, and community resource support to patients and their families. You will work closely with an interdisciplinary care team to help patients navigate medical, emotional, and social challenges while promoting positive health outcomes.
This is a community-based position requiring daily travel to patients' homes and other care settings.
Key Responsibilities
- Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, financial, and environmental needs
- Develop and implement individualized care plans in collaboration with physicians, nurses, and other healthcare professionals
- Provide counseling, education, and emotional support to patients and their families
- Coordinate community resources and connect patients with appropriate support services
- Advocate for patients to ensure they receive appropriate care and access to available resources
- Assist patients and families with discharge planning and ongoing care coordination
- Maintain accurate and timely documentation in the electronic medical record
- Collaborate with the interdisciplinary healthcare team to support continuity of care
- Manage a daily caseload while maintaining high standards of quality, professionalism, and patient-centered care
- Travel to patient homes and multiple service locations throughout the workday
Qualifications
Required
- Master's Degree in Social Work (MSW)
- Current Social Work licensure in the State of Oregon
- Strong communication, interpersonal, and documentation skills
- Ability to work independently while managing multiple priorities
- Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area

- Ability to establish professional boundaries while building strong patient relationships
- Knowledge of local community resources and support services
- Experience working in home health, hospice, case management, or healthcare settings
