Hospice RN Case Manager

Our client is seeking a Hospice RN Case Manager to provide skilled nursing care to patients in their homes while supporting patients and families through the hospice journey. This individual will serve as the primary point of contact for their assigned caseload, coordinating care with an interdisciplinary team, managing symptoms and pain, educating families, and ensuring patients receive compassionate, high-quality end-of-life care. While the primary focus is hospice, the RN may occasionally assist with home health patients as needed.

Must Haves

• Active, Oregon or California RN license.
• Graduate of an accredited nursing program.
• Current BLS certification or ability to obtain before hire.
• 1+ year of nursing experience in hospice, home health, or a healthcare facility setting.
• Valid driver's license, reliable transportation, and insurance.
• Strong communication and patient/family education skills.
• Ability to work independently in a home-based setting with limited supervision.
• Experience with clinical documentation and electronic medical records.
• Commitment to hospice philosophy and patient-centered care.

Day to Day

• Manage a caseload of hospice patients and serve as the primary nurse for assigned families.
• Conduct physical assessments and develop, implement, and revise individualized care plans.
• Participate in interdisciplinary team meetings and bi-weekly care plan reviews.
• Assess and manage pain, symptoms, medications, and overall patient comfort.
• Perform skilled nursing procedures including wound care, catheter care, IV therapy, central line care, diabetic care, and medication administration.
• Educate patients and families on diagnoses, medications, treatments, and end-of-life care.
• Coordinate care with physicians, aides, social workers, therapists, and other members of the care team.
• Complete clinical documentation, assessments, care plans, and physician orders in a timely manner.
• Supervise and delegate tasks to Hospice/Home Health Aides and other caregivers.
• Participate in quality assurance activities, care conferences, and staff training initiatives.
• Assist with home health patient evaluations, reassessments, recertifications, and discharge planning when needed.
• Provide on-call support as scheduled to assist patients and families during urgent situations.
