Job Description
Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A clinical pharmacist with specialized skills in sterile compounding is needed for a 13-week contract assignment in Garland, TX. This role focuses on managing home infusion therapy for patients, ensuring safe preparation and dispensing of sterile medications, and providing comprehensive education and support to patients and healthcare providers.
Key Responsibilities:
- Assess patient eligibility for home infusion therapy and develop personalized medication plans based on clinical and laboratory data.
- Prepare, verify, and dispense sterile medications in compliance with USP standards and regulatory requirements.
- Educate patients, caregivers, and healthcare teams on infusion therapy, medication administration, and safety protocols.
- Collaborate with healthcare providers to ensure continuity of care and monitor patient responses.
- Manage infusion devices, maintain quality assurance, and uphold safety standards.
- Provide drug information, document treatments accurately, and assist with regulatory compliance and medication safety initiatives.
Qualifications and Experience:
- Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
- Active, unrestricted Texas Pharmacist license.
- One or more of the following:
- Minimum 1 year sterile IV compounding experience,
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM),
- Completion of an ASHP-accredited Pharmacy Residency,
- Completion of Advanced Pharmacy Practice Experience (APPE),
- Equivalent pharmacy experience.
- Strong clinical knowledge of home infusion pharmacy practices.
- Ability to interpret laboratory results and apply pharmacokinetic principles.
- Excellent communication, patient counseling, and customer service skills.
