Job Description
Stellar Care and Services is hiring for a job consultant in our Supported Employment program in Colorado Springs. Our company is dedicated to providing services to individuals with developmental disabilities across the state of Colorado. The schedule for this position varies and will require evening and weekend availability.\n

\nMain Job Tasks and Responsibilities\n
\n
\nConsistently exercises individual judgment, makes decisions and solves problems.\n
\nCreate monthly progress reports to submit to DVR that include services provided and challenges the individual may face.\n
\nCompletes daily notes of individual’s progress and challenges, timesheet and goal tracking in OnTarget.\n
\nWorks with Program Director to update and maintain program participant records.\n
\nReports complaints or performance concerns; manages conflicts and disagreements in a constructive manner; responds to grievances; identifies and solves individual’s problems.\n
\nRepresent Program Director, Director or designee at internal and external meetings.\n
\nCoaches real-life job skills to individuals with disabilities at their workplace while providing the needed supports to help them obtain and maintain meaningful employment in the community.\n
\nOngoing communication with workplace supervisors, team members, and family members.\n
\nProvides on-site training, instruction, supervision, and advocacy for individuals at community job sites.\n
\nMaintains and distributes employment information such as job openings to Program Director and individuals.\n
\nSupport individuals in increasing independence in their workplace and community.\n
\nAssist individuals in understanding the functions and additional tasks of their job.\n
\nAssist individuals in communicating and cultivating meaningful relationships with coworkers and management within their job and professional community.\n
\nAssist individuals in advocating for their needs while at work, including potential formal accommodations needed.\n
\nMaintain proper community supervision requirements.\n
\nTransportation to and from individual's work/interviews, if required.\n
\nProvides medication administration to individuals served when needed under QMAP certification.\n
\nProactively performs outreach to cultivate relationships with perspective employers.\n
\nMaintains a healthy relationship with individuals serving as their single point of contact as needed; checks in regularly with the individual throughout their employment and connects them to any supports or resources needed through job placement.\n
\nCarries out duties as assigned by Program Director and/or designated supervisor.\n
\nPerforms all other duties as assigned.\n

\nQualifications:\n
\n
\nMust have reliable transportation, a valid driver's license, and a willingness to transport clients in your vehicle\n
\nPersonal vehicle use\n
\nMust be at least 18 years of age due to driving demands\n
\nMust be able to work evenings and weekend\n
\nAble to perform the physical and mental demands of the position\n
\n
\nThis is a driving position that will require the use of your personal vehicle to transport the clients we serve. The State of Colorado requires that any vehicle used for this purpose pass a detailed multi-point safety inspection through an ASE certified mechanic. Some of the items on this inspection include windshield, suspension, brakes, lights, seatbelts and all other safety features. If offered this position, Stellar Care will cover the cost of the inspection, however any repairs required are at the expense of the applicant and must be resolved within 30 days of hire. Vehicles 5 years or newer are exempt from this inspection.\n
