RN Case Manager

Our organization, committed to providing coordinated healthcare solutions for residents of Connecticut, is on the lookout for a Registered Nurse Case Manager to join our PACE Home Care initiative. This position involves overseeing a group of participants, performing in-home evaluations, and creating as well as managing personalized care plans.

As a key member of an interdisciplinary team, you will:

• Conduct thorough assessments in participants' homes.
• Develop and coordinate tailored care strategies.
• Work collaboratively with various healthcare professionals to enhance patient outcomes.
• Educate both patients and their families about care processes.
• Facilitate seamless transitions in care.
• Connect participants with essential community resources.
