\nLutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate, organized, and self-motivated IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.
\nThis position combines elements of case management, care coordination, social work, disability services, long-term care, and community-based support to help participants maintain independence, achieve personal goals, and direct their own services.
\nAs an IRIS Consultant, you will build meaningful relationships with participants, coordinate services, promote self-determination, and ensure health and safety while supporting participants in living the life they choose.

\nWhat You'll Do
\nAs an IRIS Consultant, you will:
\n- \n
- Develop and maintain trusted relationships with IRIS participants \n
- Assist participants in creating and updating individualized IRIS Support and Services Plans \n
- Conduct monthly participant contacts and required quarterly in-person visits \n
- Promote participant choice, self-direction, independence, and community inclusion \n
- Monitor participant health, safety, and program outcomes \n
- Educate participants about IRIS enrollment requirements, eligibility, and available resources \n
- Collaborate with Aging and Disability Resource Centers (ADRCs), healthcare providers, community organizations, and service providers \n
- Maintain accurate and timely case documentation within required systems \n
- Complete documentation within established compliance standards and timelines \n
- Assist with annual Long-Term Care Functional Screens when applicable \n
- Work independently while collaborating with an interdisciplinary support team \n
Qualifications
\nRequired
\nOne of the following:
\n- \n
- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Sociology, Special Education, Nursing, Rehabilitation Services, Healthcare Administration, or a related field and at least one year of supervised experience working with older adults or individuals with disabilities \n
OR
\n- \n
- Four (4) or more years of direct experience providing services to older adults or individuals with disabilities \n
OR
\n- \n
- Current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption \n
