Position Overview
- Employment Type: Full-time (1.0 FTE)
- Office Hours: Monday to Friday, 8 AM to 5 PM
- Location: 6050 Northland Dr Ste 200, Rockford, MI 49341
- Specialization: Family Medicine Physician, outpatient

Team Composition: 8 Family Medicine physicians, 2 Pediatricians, 1 Family Medicine Nurse Practitioner, and 2 Pediatric Nurse Practitioners
- Clinical Care Team: 2 providers and 3 Medical Assistants/LPNs
- Teaching Opportunities: Yes - Involvement with Residents, Medical Students, and Fellows
- On-Call Requirements: Phone call coverage one week every two months, from 5 PM to 12 AM on weekdays, and 7 AM to 7 PM on weekends
- Additional Information: Panel to be developed immediately.

Qualifications
- Welcoming new graduates
- Board certified in the relevant clinical specialty or must achieve board certification within the timeline set by the applicable specialty board.
- Possession of an active Michigan medical license (or the ability to obtain one)
- DEA license required
Responsibilities
A typical day may involve:
- Delivering direct patient care
- Collaborating with multidisciplinary teams
- Engaging in teaching and mentorship
- Documenting patient information in Epic EMR
- Participating in quality improvement initiatives
- Supporting community health initiatives
