Care Coordinator for a Day Centre
\nSalary: £27,000 - £30,000 | Location: South Tyneside | Type: Full-time, Permanent
\nWe are recruiting on behalf of a values-led care organisation for a Care Coordinator who will be responsible for coordinating and supporting the delivery of high-quality, person-centred services for adults with learning disabilities, attending the day centre. The role involves developing support plans, liaising with families and external professionals, coordinating activities and services.
\nKey Responsibilities
\nCare Planning and Coordination

- Maintain care plans, risk assessments, and care records. \n
- Ensure compliance with CQC, safeguarding, GDPR, and health & safety standards. \n
- Support audits, quality assurance, and continuous improvement initiatives. \n
- Liaise with service users, families, professionals, and external agencies. \n
- Investigate and record incidents, complaints, and safeguarding concerns. \n
- Provide direct care support when required. \n
What We're Looking For
\n- \n
- Experience in care coordination, administration, or support services. \n
- Knowledge of care planning, safeguarding, and CQC requirements. \n
- Strong IT skills, including Microsoft Office and digital care systems. \n
- Organised, detail-focused, and able to manage multiple priorities. \n
- Full UK Driving Licence and willingness to complete an Enhanced DBS check. \n
