Venn Group are recruiting for a Community Support Worker on behalf of a well-established support provider in Liverpool.

This is an excellent opportunity for an experienced support professional who is passionate about helping vulnerable individuals live independently within the community. You will manage a caseload of approximately 12 service users, providing tailored support to individuals with complex needs who are on their recovery journey.

Key Responsibilities

• Managing a caseload of around 12 clients living within the community.
\n
• Supporting individuals to maintain their independence and achieve positive outcomes.
\n
• Developing and reviewing support plans and risk assessments.
\n
• Providing practical and emotional support to clients with complex needs.
\n
• Accompanying service users to appointments and external meetings.
\n
• Working closely with partner agencies and support services to coordinate effective interventions.
\n
• Maintaining accurate case notes and support records.
\n
• Promoting wellbeing, tenancy sustainment, independence and community engagement.
\n

Essential Requirements

\n
• Experience supporting vulnerable adults with complex needs.
\n
• Understanding of recovery-focused support and person-centred approaches.
\n
• Strong communication, organisation and time management skills.
\n
• Confidence in lone working and managing your own caseload.
\n
• Enhanced DBS covering both Adults and Children.
\n
• Full UK driving licence and access to a vehicle is highly desirable.
