Concord Hospital Medical Group is seeking an experienced APRN or PA-C to join our Geriatric Program, providing primary care to older adults in their homes and nursing facilities.
\nThis unique 4-day work week offers a blend of independence and collaboration, with two days spent caring for patients in their homes and two days providing care in nursing facility.

What You'll Do
\n- \n
- Provide primary care to older adults in their homes and nursing facilities \n
- Assess, diagnose, and manage acute and chronic medical conditions \n
- Order and interpret diagnostic testing \n
- Prescribe and manage medications \n
- Educate patients and caregivers on disease management, prevention, and safety \n
- Partner with physicians, residents, nurses, and interdisciplinary team members to deliver high-quality care \n
- Support Family Medicine residents in the care of geriatric patients \n
- Document care and maintain compliance with clinical and regulatory standards \n
Qualifications
\n- \n
- Master's degree from an accredited APRN or Physician Assistant program \n
- Current or eligible New Hampshire APRN or PA-C licensure \n
- Current DEA registration \n
- Minimum of two years of post-graduate clinical experience in geriatrics, primary care, home-based care, or nursing facility medicine \n
- Experience providing care in home-based and/or nursing facility settings strongly preferred \n
- Demonstrated interest in caring for older adults with complex medical needs \n
- Strong clinical assessment, communication, and interdisciplinary collaboration skills \n
