Job Description

Established in 2007, Select Lifestyles Limited provides support to adults with learning disabilities and complex needs within our residential homes, supported living schemes and day centres across the West Midlands.

Job Purpose

\n

Our Care Co-ordinators carry overall responsibility for each individual service user and their service, including health and safety responsibilities in the absence of the Manager.

\n

Care Co-ordinators are responsible for the effective management of their service, ensuring that the staff team provides high-quality support to individuals with learning disabilities, including those who may present behaviours that challenge. This support should enable service users to participate in everyday activities and fulfil daily living functions safely and confidently.

\n

The Care Co-ordinator must ensure that staff promote personal independence, informed choice, and the opportunity for service users to live the lifestyles they wish to lead. The Care Co-ordinator and their team are expected to demonstrate a values-based approach, characterised by respect, dignity, inclusion, and unconditional positive regard at all times.

\n

Responsibilities

\n

\n

The Care Co-ordinator is personally responsible for ensuring they have a thorough knowledge of each individual service user’s mental and physical condition. Ensuring all staff are recording observations satisfactorily with appropriate content and description. Regular auditing of observations must be undertaken.

\n

\n

To manage high standards of personalised care for individual service user’s within their home. To supervise staff performance in respect of support workers and any other members of the staff team. Ensuring that adequate precautions are taken to ensure the safety of both service users and staff.

\n

\n

Take the lead during emergency situations, assist, and relieve staff as required. Conduct investigation as to why incident occurred and provide reports or attend de-brief with Registered Manager, Operations/Locality Manager or other member of the Senior Management Team. Ensure compliance with CQC regarding reporting of such incidents and liaison with Responsible Individual.

To provide feedback to the Operations Management team

\n

regarding service affairs including staff performance, reporting of incidences, sharing knowledge or making suggestions to improve service.

\n

\n

Monitor Staff performance and provide mentorship to support all staff, as required. Undertake regular supervisions with staff ensuring that the team leaders are mentored at least by the Supported Living Service Manager.

\n

\n

To initiate staff appraisals and ensure all staff are annually appraised.

\n

\n

To ensure attendance at monthly management meetings, supervisions and appraisals ensuring they have staffed the service accordingly to facilitate attendance.

\n

\n

Ensure all staff receive a full induction with internal training and completion of Induction booklets.

\n

\n

Manage staff in accordance with Company policies and procedures by including but not limited to;

\n

\n

Absence reporting and recording (including own)

\n

\n

Safeguarding

\n

\n

Whistleblowing

\n

\n

Ensure effective handovers are undertaken by staff from shift to shift.

\n

\n

Must check rotas on an ongoing basis to ensure all shifts maintain safe staffing levels and where it is evident that this is not the case organise staffing and inform the Operational Management team as to what has occurred.

\n

\n

Manage staffing levels on shift and if necessary, due to emergency absence or absence at short notice liaise with first and second on-call.

\n

\n

Undertake on-call duties as identified on the on-call rota. Any request to change the on-call rota must be authorised by the Operations Manager.

\n

\n

Ensure production and review of personalised care plans, risk assessments and protocols for service users and employees.

\n

\n

Undertake assessments of service user needs and manage transition process for prospective new service users.

\n

\n

Each service user must have a written contract or statement of terms and conditions and those terms are fulfilled.

\n

\n

Liaise fully with multidisciplinary agencies in the provision of care to the service user.

\n

\n

Communicate effectively with Operations Manager and Senior

\n

Management team, keeping them fully appraised as to the service user’s needs and behaviour.

\n

\n

To facilitate good systems of good communication between service users, relatives, staff and multi-disciplinary agencies.

\n

\n

Ensure complaints by or on behalf of service users is dealt with in accordance to Company Complaints procedure and is reported to the Operational Management team immediately.

\n

\n

Attend meetings with the operations manager to provide verbal and written reports (as required) regarding the performance of service and care of service users.

\n

\n

Identified budgets must be managed effectively.

\n

\n

Ensure CQC compliances and standards are fully implemented and maintained at all times.

\n
