Job Description

As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.

Schedule: Monday - Friday with flexible scheduling, 32 patient contact hours minimum. Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call

\n

EMR: Athena

\n

On-Call: Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up.

Requirements:

  • \n

    Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA).

    Unrestricted DEA licensure and prescribe authority (per state practice regulation).

    At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred. If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification. Responsibilities:

    \n
  • \n

    Practice exceptional medicine within knownwell

  • \n
  • \n

    Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only. Share a panel with MD/DO + other APCs.

  • \n
  • \n

    Embodies the knownwell mission and values of patient inclusivity in how you deliver care.

  • \n
  • \n

    Coordinates with the broader care team to ensure the patient has the support they need.

  • \n
  • \n

    Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.

  • \n
  • \n

    Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.

  • \n

    See adolescents ages 12+ for metabolic health and primary care.

  • \n
  • \n

    Ensure panel is open to any individual, regardless of their current health status and comorbidities.

  • \n
  • \n

    Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. Provide an environment in which patients feel comfortable and free from judgment.

    Offer exceptional encouragement as patients reach their goals.

  • \n
  • \n

    Coordinate across the collaborative care team

  • \n
  • \n

    Escalate any concerns to your dedicated site leadership team.

  • \n
  • \n

    Provide clinical guidance to medical assistants and nursing teams as assigned.

  • \n
  • \n

    Stay current with licensure and appropriate re-certifications.
