Our facilities operate as Skilled Nursing Facilities (SNFs) that offer:
- Long-term care: Support for residents needing ongoing skilled assistance and compassionate daily care.
- Short-term rehabilitation: Services for patients recovering from surgery, illness, or injury.

Position SummaryThe Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Qualifications- Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT); those graduating in 2003 or later must possess an MSPT or DPT.
- New graduates are encouraged to apply.
- Active participation in community health initiatives/projects as appropriate.
- Proficiency in Microsoft Word, Excel, and email communication.
- Engage with patients, families, and support departments effectively.
- Understand and comply with the laws, regulations, and guidelines relevant to post-acute care facilities.
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
- Maintain confidentiality of all proprietary and sensitive information.
- Adhere to organizational policies, including those related to harassment and compliance.
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
- Follow established physical therapy treatment plans for patients.
- Supervise and assess the care provided by Physical Therapist Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
- Utilize therapy software accurately and effectively.
- Provide excellent customer service and maintain a positive attitude towards patients.
- Read, write, speak, and comprehend the English language proficiently.
