HarmonyCares is seeking a Nurse Practitioner to join our home-based primary care team in the Lancaster, PA area. This full-time role offers the opportunity to deliver high-quality, relationship-based care to medically complex seniors in their homes.

As a physician-led organization with 30+ years of experience, we focus on outcomesnot volume, giving providers the time and support to practice meaningful medicine.

610 patients/day with up to 60-minute visits
Smaller patient panel with a focus on continuity and outcomes
Autonomy in the field with strong interdisciplinary support
MondayFriday schedule with limited telephonic call

Provide primary care in a residential setting, including assessment, diagnosis, treatment, documentation, and coordination with the care team.

Required:
\n Active, unrestricted NP license (PA) + DEA
\n Board Certified/Eligible
\n CPR certification
\n Ability to travel locally

Preferred:
\n Geriatric or complex care experience
\n Strong collaboration and communication skills
