We are looking for a dedicated Certified Occupational Therapy Assistant to join our team in the PACE program, focusing on enhancing the quality of life for seniors. In this role, you will play a vital part in executing treatment plans, facilitating therapeutic activities, and assisting participants in preserving their independence in both home and community settings.

Key Responsibilities
  • Implement individualized treatment plans tailored to each participant's needs.
  • Lead engaging therapeutic activities that promote physical and mental well-being.
  • Support participants in maintaining their independence with daily tasks.
  • Collaborate closely with an Occupational Therapist and a multidisciplinary care team.
  • Document participant progress and adjust interventions as necessary.
  • Provide education and support to participants and their caregivers.
Qualifications and Requirements
  • Certification as an Occupational Therapy Assistant.
  • Experience working with older adults is preferred.
  • Strong communication and interpersonal skills.
  • Ability to work effectively within a team environment.
