\n Value Based Care Model - delivering primary care to geriatric panel, Monday - Friday, no weekends or evenings.\n

\n Manage chronic conditions of our own patient panel seeing 6-8 patients per day on average. Start day at 8AM with a quick stand-up huddle, ending day around 4:30 p.m. taking no charting in to the evenings using Athena and AI Scribe.\n

\n The APPs we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APP who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.\n

\n Qualifications:\n

  • \n
    \n Licensed as a NP or PA in the State where HarmonyCares Medical Group office located\n
    • \n
  • \n
    \n Federal DEA License/eligibility\n
    • \n
  • \n
    \n Geriatric training/experience preferred\n
    • \n
  • \n
    \n Active CPR/BLS Certification\n
    • \n
  • \n
    \n Board Certification\n
    • \n
