Position Overview
\nWe are seeking an experienced Licensed Master Social Worker (LMSW) to provide inpatient social work services within a hospital setting in New York, NY. This role focuses on conducting psychosocial assessments, providing supportive counseling, coordinating discharge planning, and connecting patients and families with appropriate community resources.
\nThe ideal candidate will have recent acute care hospital experience and a strong background working with inpatient medicine, surgery (med/surg), and rehabilitation patient populations.
\nResponsibilities

- Conduct comprehensive psychosocial assessments for hospitalized patients. \n
- Provide supportive therapy and emotional support to patients and families. \n
- Develop and implement discharge plans to ensure safe transitions of care. \n
- Coordinate referrals to community resources, rehabilitation programs, home care services, and other support systems. \n
- Collaborate with physicians, nurses, case managers, and interdisciplinary care teams. \n
- Assist patients and families in navigating healthcare systems and available services. \n
- Document assessments, interventions, and discharge planning activities in the medical record. \n
- Advocate for patient needs and support continuity of care. \n
Qualifications
\nRequired Education
\n- \n
- Master's Degree in Social Work (MSW) \n
Required Licensure
\n- \n
- Active New York Licensed Master Social Worker (LMSW) \n
Required Experience
\n- \n
- Minimum of 1 year of hospital, acute care, or inpatient LMSW experience \n
- Required experience must have occurred within the past 2 years \n
- Experience working with inpatient medicine, surgery (med/surg), and rehabilitation populations \n
Preferred Skills

- Strong psychosocial assessment and counseling abilities \n
- Knowledge of discharge planning and care coordination processes \n
- Familiarity with community resources and referral networks \n
