Key Responsibilities

• Conduct comprehensive assessments to evaluate patient needs.
• Develop, implement, and monitor individualized care plans.
• Collaborate with healthcare providers to facilitate safe and effective transitions of care.
• Advocate for patients’ needs and preferences.
• Support patients in managing diseases and medications.
• Encourage preventive care practices among members.

Qualifications and Requirements

• Current RN licensure is mandatory.
• Experience in home health or case management is essential.
• Preferred: Background in managed care settings.
