Our facilities operate as Skilled Nursing Facilities (SNFs) that offer:

Position Summary

The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Qualifications and Experience

• Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT); those graduating in 2003 or later must hold an MSPT or DPT.
• Current registration or licensure in the applicable state, with an active license in good standing throughout employment.
• Preferred one year of experience; however, new graduates are encouraged to apply.
• CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance of certification throughout employment.

Job Responsibilities

• Engage in community health initiatives and projects as appropriate.
• Proficiently use Microsoft Word, Excel, and email.
• Coordinate with patients, families, and support departments to effectively address patient needs.
• Exhibit proper body mechanics at all times.
• Make independent decisions when necessary.
• Understand and comply with practices, procedures, laws, and regulations governing post-acute care facilities.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
• Maintain professional relationships with colleagues, vendors, and others.
• Ensure confidentiality of all proprietary and sensitive information.
• Adhere to company policies, including those related to harassment and compliance.
• Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.

Essential Functions

• Implement physical therapy treatment plans for patients.
• Set, assess, and adjust realistic, measurable, and functional goals.
• Supervise and evaluate care provided by PTAs, Rehab Aides, and PT and PTA students.
• Document care accurately and in a timely manner.
• Utilize therapy software effectively and accurately.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Assist in the safe evacuation of patients when necessary.
• Demonstrate reliable attendance and punctuality.
• Utilize reasoning skills and sound judgment in various situations.
• Collaborate effectively within an interdisciplinary team.
• Perform physical tasks such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
• Read, write, speak, and comprehend the English language.
