- MD or DO degree from an accredited institution.
- Completion of an ACGME-accredited OB/GYN residency (or expected completion prior to start).
- Board certified or board eligible in OB/GYN.
- Eligibility for licensure in Oregon.
- Eligibility for DEA and hospital privileges.
- Insurability for malpractice coverage.
