Primary Care, Primary Care Nurse Practitioner - See 5-9 Patients per day | Reading, PA area

HarmonyCares is seeking a Nurse Practitioner to join our home-based primary care team in the Reading, PA area. This full-time role offers the opportunity to deliver high-quality, relationship-based care to medically complex seniors in their homes.

As a physician-led organization with 30+ years of experience, we focus on outcomes—not volume, giving providers the time and support to practice meaningful medicine.

• 6–10 patients/day with up to 60-minute visits
\n • Smaller patient panel with a focus on continuity and outcomes
\n • Autonomy in the field with strong interdisciplinary support
\n • Monday–Friday schedule with limited telephonic call

Provide primary care in a residential setting, including assessment, diagnosis, treatment, documentation, and coordination with the care team.

Required:
\n • Active, unrestricted NP license (PA) + DEA
\n • Board Certified/Eligible
\n • CPR certification
\n • Ability to travel locally

Preferred:
\n • Geriatric or complex care experience
\n • Strong collaboration and communication skills
