Qualifications (What You Bring)
- Graduate from an accredited nursing program; a Bachelor’s degree is preferred.
- Active RN license in the state of Colorado.
- At least two (2) years of nursing experience, including long-term care, dementia care, or acute care; home care experience is a plus.
- CPR certification is required.
Key Responsibilities
As a Registered Nurse (RN) , you will manage cases, coordinate care, supervise, and implement professional and supportive services for clients, utilizing nursing theories and processes in line with the Nurse Practice Act.
- Evaluate client referral information and respond to inquiries as needed.
- Conduct client assessments according to agency standards to ensure effective home care services.
- Independently direct, plan, and initiate appropriate actions in home care scenarios.
- Carry out job responsibilities in accordance with agency policies, procedures, and community standards.
- Perform additional duties as assigned by the Director of Clinical Services.

Experience Required
- 2 years of nursing experience, including long-term care dementia care or acute care
- Experience in home care.
- Bachelor’s degree in Nursing or a related field
- CPR/First Aid certification
- Registered Nurse license
