Job Opportunity: Home Health Registered Nurse

Our organization is in search of a dedicated and empathetic Home Health Registered Nurse to provide expert nursing care within the homes of our patients. The ideal candidate will be responsible for conducting thorough assessments, administering medications and treatments, managing wounds and intravenous therapies, and creating personalized care plans tailored to each patient’s needs.

Key Responsibilities:

• Conduct comprehensive patient assessments.
• Administer medications and treatments as prescribed.
• Manage wound care and intravenous therapies.
• Develop and implement individualized care plans.
• Ensure precise documentation in OASIS and electronic health records (EHR).
• Educate patients and their families on care procedures and health management.
• Collaborate closely with physicians and a multidisciplinary team.

Qualifications and Requirements:

• Current RN license.
• Experience in home health care is preferred.
• Strong assessment and clinical skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and as part of a team.
