The Physical Therapist I is responsible for providing evidence-based, patient-focused rehabilitation services aimed at restoring function, enhancing mobility, alleviating pain, and promoting independence. You will work closely with physicians and a multidisciplinary care team to create personalized treatment plans that support patients in reaching their rehabilitation objectives.
- Assess patients and formulate tailored treatment plans.
- Administer therapeutic exercises, manual therapy, gait training, mobility assistance, and functional rehabilitation.
- Track patient progress and adjust treatment plans as necessary.
- Educate patients and their caregivers about treatment strategies and home exercise routines.
- Collaborate with physicians and interdisciplinary teams to enhance patient outcomes.
- Maintain precise EMR documentation and adhere to hospital and regulatory standards.
- Degree in Physical Therapy (Bachelor's, Master's, Doctorate, or Certificate).
- Valid state Physical Therapy license.
- Current BCLS (AHA) certification upon hire, with maintenance required.
