Nurse Practitioner

Our organization is on the lookout for a dedicated Nurse Practitioner who is passionate about delivering primary and preventive healthcare to seniors participating in our PACE program. This role is interdisciplinary, allowing you to:

• Conduct thorough assessments and evaluations
• Diagnose and manage both acute and chronic health conditions
• Prescribe necessary medications
• Create personalized care plans tailored to individual needs
