POSITION SUMMARY

Our Nurse Practitioners (NPs) and Physician Assistants (PAs) are essential contributors to our organization’s success, assisting our partners in improving the quality of life for their residents. Key responsibilities include:

  • Evaluating patients’ medical and mental health requirements and developing a proactive care plan that addresses their conditions and any obstacles to receiving care.
  • Ensuring adherence to regulations set by local, state, and federal agencies concerning the clinical services provided.
  • Prescribing medications and arranging lab tests, diagnostic procedures, and consultations.
  • Tracking patients’ adherence to treatment plans and their responses, making necessary adjustments as needed.
WORK EXPERIENCE, CREDENTIALS, AND EDUCATION
  • Current APRN or PA license is mandatory.
  • Degree from an accredited APRN or PA program is required.
  • A minimum of three years of experience in clinical nursing or rehabilitation, particularly with geriatric populations or in fee-for-service settings.
  • Experience with Electronic Health Records is essential.
  • Proficiency in Microsoft Office applications, including Word, Outlook, and Excel.
SUPERVISORY RESPONSIBILITIES
  • May involve providing training and guidance to facility staff.

Education
  • Bachelor's degree or higher preferred.
