Job Description
Provide outpatient primary care for a Medicare-focused patient population with a weekday-only schedule and no call. This assignment is ideal for physicians who enjoy managing chronic conditions in an established clinic setting.

- Average of 16 to 18 patients per day
Practice & Cases
- Outpatient primary care for adults primarily age 65 and older
- Manage diabetes, hypertension, hyperlipidemia, CHF, coronary artery disease, and mild cognitive impairment
- Nurse Practitioner, Medical Assistant or scribe, and Registered Nurse support
- No procedures required
Schedule
- Monday through Friday
- 8:00 a.m. to 5:00 p.m.
- Weekday-only clinic coverage
- Outpatient practice with no call
Requirements
- Must be an MD or DO
- Active Illinois medical license required
- Board Certified in Internal Medicine or Family Medicine
- DEA required and Spanish fluency required
