Key Responsibilities of the Nurse Case Manager:- Medical case management
- Conducting screenings
- Providing education
- Offering counseling services
- Managing medication
- Collaborating with community healthcare providers
- Participating in a rotating on-call schedule, primarily offering remote consultations to ensure accessibility without needing to be on-site
- Bachelor of Science in Nursing or Associate of Science in Nursing
- Completion of an accredited nursing program
- Current Registered Nurse licensure in the State of Illinois and a valid Illinois Driver's License
- Preferred experience in psychiatric care with children and adolescents
- Excellent organizational, communication, and clinical abilities
