Our facilities function as Skilled Nursing Facilities (SNFs), offering:

Position SummaryThe Physical Therapist (PT) delivers direct rehabilitation care in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Qualifications and Experience- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must possess an MSPT or DPT)
- Currently registered or licensed in the applicable state, with an active license in good standing throughout employment
- Preferred one year of experience; however, new graduates are welcome to apply
- CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance of certification during employment
- Engage in community health initiatives and projects as appropriate
- Proficiently use Microsoft Word, Excel, and email
- Coordinate with patients, families, and support departments to effectively address patient needs
- Exhibit proper body mechanics at all times
- Make independent decisions when necessary
- Understand and comply with practices, procedures, and regulations governing operations in a post-acute care facility
- Implement and interpret departmental programs, goals, objectives, policies, and procedures
- Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
- Maintain professional relationships with colleagues, vendors, and others
- Safeguard the confidentiality of all proprietary and confidential information
- Adhere to company policies, including those related to harassment and compliance
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training
- Implement physical therapy treatment plans for patients
- Set, evaluate, and adjust realistic, measurable, and functional goals
- Supervise and assess care provided by PTAs, Rehab Aides, and PT and PTA students
- Document care accurately and in a timely manner
- Utilize therapy software effectively
- Provide excellent customer service and maintain a positive attitude towards patients
- Assist in the safe evacuation of patients when necessary
- Demonstrate reliable attendance
- Utilize reasoning skills and sound judgment
- Communicate effectively and collaborate within an interdisciplinary team
- Engage in various physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving throughout working hours
- Read, write, speak, and comprehend the English language
